This guide cuts through the confusion, explains exactly what is happening with NHS access right now, and explains how a trusted, GPhC-registered online pharmacy like Happy Pharmacy can provide safe, regulated, and personal access to weight loss injections — often within days.

If you have been looking into weight loss injections, you will almost certainly have come across two names: Mounjaro and Wegovy. Both are MHRA-approved treatments delivering real, clinically evidenced results. And both are now available on the NHS — in theory. In practice, the picture is far more complicated, and for the vast majority of people in the UK, the NHS route is either years away or simply out of reach. So what are your options?

The NHS Promise — and the Reality

The NHS has committed to rolling out Mounjaro (tirzepatide) as a weight management treatment across England, following NICE approval. From 23rd June 2025, GPs became able to prescribe it — and from 1st April 2026, prescribing of tirzepatide for obesity was formally incorporated into the new GP Quality and Outcomes Framework (QOF) contract.

That sounds positive. But participation is optional for GP practices, eligibility criteria are extremely strict, and access varies dramatically depending on where you live. NHS England's own phased rollout targets just 220,000 patients nationally over the first three years — in a country where an estimated two million people are already paying privately for GLP-1 medications.

NHS Eligibility for Mounjaro (as of 2026)

BMI of 40 or above — or 37.5+ for South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds

At least one weight-related comorbidity (such as type 2 diabetes, hypertension, or cardiovascular disease)

Referral to a specialist weight management service or GP participating in the QOF scheme

The Scale of the Challenge — By the Numbers

The gap between NHS ambition and real-world access is stark. A UCL study published in BMC Medicine in January 2026 revealed just how large the private market for weight loss medications has grown in the UK.

These figures make the picture clear. The overwhelming majority of people accessing weight loss injections in the UK are doing so privately — and Mounjaro now dominates that market. The challenge is that not all private providers are equal, and the explosion in demand has unfortunately attracted unsafe, unregulated sellers.

The Danger of Going Outside Regulated Channels

An Ipsos poll conducted at the start of 2026 found that one in ten people said they would consider buying weight loss jabs via social media. NHS England's national medical director responded directly, warning that unverified sellers offer medicines without clinical oversight, medical checks, or follow-up care — and that the products themselves may be fake, contaminated, or incorrectly dosed.

The MHRA has echoed these concerns, highlighting that illegal sellers can cause serious harm — and that weight loss injections are powerful medicines requiring professional clinical oversight at every stage. This is not a minor regulatory technicality. It is the difference between safe, effective treatment and genuine physical risk.

Mounjaro vs Wegovy: Which Injection is Right for You?

Both Mounjaro and Wegovy are weight loss injections licensed by the MHRA for chronic weight management in adults with obesity or overweight and at least one weight-related health condition. They work differently, however, and produce different average results in clinical trials.

Mounjaro (tirzepatide) is a dual GIP/GLP-1 receptor agonist. By targeting two hormonal pathways simultaneously, it has demonstrated exceptional results in trials — the SURMOUNT-1 study found average weight reductions of around 22% of body weight at the highest dose over 72 weeks. Wegovy (semaglutide) is a GLP-1 receptor agonist that produced average weight loss of around 15% in the STEP 1 trial. Both deliver meaningful, life-changing weight loss results when combined with lifestyle support.

The right choice depends on individual clinical factors, your medical history, and what a registered prescriber considers appropriate for you — which is precisely why a personal clinical consultation matters.

Weight Regain After Stopping — Understanding the Risks

One of the most important recent developments in this field was a large review published in the British Medical Journal in January 2026, led by researchers at the University of Oxford and funded by the NIHR. The study — which examined 37 trials involving more than 9,000 participants — found that people who stop weight loss injections can regain weight up to four times faster than those who used traditional dieting and exercise.

This underlines something that every reputable provider should be communicating clearly: weight loss injections are not a short-term fix. They are a medical treatment that works best as part of a sustained, supported programme. At Happy Pharmacy, that ongoing personal support is central to the service — because real results depend not just on starting treatment, but on staying safely on track.
